Low Testosterone Doesn't Always Look Like
What You'd Expect
Most men expect a dramatic decline. What actually happens is quieter — and easy to dismiss. Fatigue you chalk up to stress. Weight that won't move. A general flatness where drive used to be. These aren't just signs of getting older. They may be signs that your testosterone levels are no longer where they need to be.
Feeling tired even after a full night's sleep, or running out of steam well before the day is over.
Training consistently but not seeing results — or losing muscle mass you worked years to build.
Excess weight that accumulates around the midsection and resists diet and exercise changes.
Reduced interest in sex or physical intimacy — often accompanied by changes in performance or confidence.
Difficulty concentrating, slow recall, or a general sense of mental dullness that wasn't there before.
Shorter fuse, low motivation, or a creeping sense of anxiety or depression without an obvious cause.
If several of these sound familiar, you're not alone. Testosterone levels decline gradually in most men after age 30. The good news: low T is diagnosable with a simple blood panel, and treatable. The first step is a conversation.
What to Expect When You Come to Us
Starting TRT at Body Balance Medical is straightforward. We handle the evaluation, the lab work, and the ongoing monitoring — so you can focus on how you feel.
Meet with Mia, our hormone and longevity NP, to discuss your symptoms, health history, and goals. In-person or virtually.
A comprehensive blood panel measures your total and free testosterone, estrogen, thyroid markers, and other key hormonal indicators.
Your provider reviews your results and builds a therapy plan matched to your labs, your symptoms, and your lifestyle — not a one-size-fits-all template.
Regular follow-up labs and check-ins keep your levels dialed in over time. We're with you for the long game — not just the first prescription.
TRT Delivered the Way That Works For You
There's no single best delivery method for testosterone — the right choice depends on your labs, your lifestyle, and how involved you want to be in day-to-day management. Your provider will walk you through the options.
The most widely used method. Typically administered weekly or biweekly — in-clinic or self-administered at home. Provides reliable, consistent dosing.
A smaller needle injected into fatty tissue just under the skin. Many patients find this more comfortable and easier to self-administer.
Applied daily to the skin — no needles required. Levels tend to be more stable day-to-day, though absorption can vary between individuals.
Small pellets inserted under the skin that dissolve gradually over 3–6 months. No daily or weekly dosing. Mia is Biote-Certified and experienced in pellet protocols.
The Science on TRT and Heart Health Has Changed — Significantly
For years, testosterone therapy carried an FDA black-box warning linking it to elevated cardiovascular risk. That warning has been removed. Here's what changed.
The TRAVERSE trial was the largest randomized controlled trial ever conducted on testosterone therapy and cardiovascular outcomes — nearly 5,200 men, followed for an average of 33 months.
The findings: testosterone therapy did not increase the risk of major cardiovascular events in men with hypogonadism. Based on this evidence, the FDA removed its black-box cardiovascular warning from testosterone therapy labeling in February 2025.
This doesn't mean TRT is without considerations — every patient's cardiovascular history still matters. But the long-standing concern about heart risk is no longer supported by the best available evidence.
TRT and Fertility — A Conversation Most Providers Skip
Testosterone replacement therapy suppresses the body's natural production of testosterone, which also affects sperm production. For men not currently planning to father children, this is typically manageable. For men who are, it's something we need to discuss before starting any protocol.
Standard TRT protocols are well-established and generally well-tolerated. Ongoing monitoring keeps your overall hormonal picture balanced throughout treatment.
There are alternative approaches — including clomiphene and hCG — that can raise testosterone while preserving or supporting sperm production. We'll walk you through the options.
We raise this because you deserve a complete picture — not a sales pitch. Whether TRT is the right path or an alternative approach makes more sense for your situation, we'll help you understand your options and make an informed decision.
Mia LoPreiato, NP
Mia leads hormone optimization at Body Balance Medical, including testosterone replacement therapy for both men and women. She's Biote-Certified — a specialized designation for hormone pellet therapy — and takes a longevity-focused approach to every protocol she designs.
Hormone therapy isn't a one-size-fits-all protocol. Mia starts with understanding where you are — your labs, your symptoms, your lifestyle — and builds from there. Ongoing monitoring is part of the process, not an afterthought.
What Most People Want to Know Before They Book
The only way to confirm low testosterone is through a blood test — specifically a measurement of total and free testosterone levels. Symptoms alone can be misleading since they overlap with other conditions. At Body Balance Medical, we start with a lab panel as part of your initial evaluation, so you're working with actual numbers, not guesses.
TRT is considered safe for appropriately selected candidates under proper medical supervision. The TRAVERSE trial (2023) — the largest randomized controlled trial on the subject — found no increased risk of major cardiovascular events in men with hypogonadism on TRT. The FDA removed its black-box cardiovascular warning in February 2025 based on this evidence.
Like any medical therapy, TRT has potential side effects and isn't appropriate for everyone. Your provider will review your health history, current medications, and labs before recommending any protocol.
Exogenous testosterone suppresses the hormones that stimulate sperm production, which can reduce sperm count during treatment. If you're not planning to have children, this is generally manageable and often reversible.
If you are planning a family, there are alternative approaches — including clomiphene citrate and hCG — that can support testosterone levels while preserving fertility. Your provider will discuss this before any protocol begins.
Results vary by symptom and individual. Many patients notice improvements in energy and mood within a few weeks. Changes in body composition, libido, and strength typically become more apparent over two to three months. Full optimization — where levels are dialed in and stable — usually takes three to six months, which is why consistent follow-up labs are part of the process.
Not necessarily. Depending on your delivery method, many patients self-administer injections at home after a brief training. Pellet therapy only requires a short in-office procedure every three to six months. Topical gels are applied daily at home. Your provider will recommend the option that fits your schedule and make sure you're fully comfortable with whatever approach you choose.
Yes and no — it depends on which part of the process you're asking about. The lab panels we use to evaluate and monitor your hormone levels are billed through insurance, so most patients with coverage pay little or nothing for bloodwork.
The TRT program itself — your monthly treatment plan, provider visits, and medication — is cash pay. Payment plans are available through Cherry Financing and CareCredit for patients who prefer to spread costs over time. Contact our office for current program pricing and we'll walk you through what to expect before you commit to anything.
