Does Tirzepatide Lower Testosterone? What the Evidence Actually Shows

Clinician discussing tirzepatide and testosterone questions with a male patient

Body Balance Medical·12 min read·Hormone Optimization·Summerlin, Las Vegas

Short answer

No credible evidence shows tirzepatide directly lowers testosterone. In men with obesity, the opposite pattern shows up more often: as body fat drops, testosterone tends to rise. The data is still early, mostly short-term, and drawn from small studies. Libido and erectile changes are possible but are not established side effects.

If you searched this question because your labs shifted, or because something has felt different since starting treatment, you deserve a straight answer instead of a forum thread. Low testosterone linked to obesity is common, well documented, and often improves with weight loss. Tirzepatide's relationship to it is more nuanced than a simple raises-it-or-lowers-it framing allows, especially for men already running TRT and GLP-1 therapy together.

This post walks through what the current research actually says, in men and in women, and where the honest answer is still "we don't know yet."

How Tirzepatide Could Affect Testosterone

Tirzepatide is a dual GIP and GLP-1 receptor agonist. It was not designed to act on the hypothalamic-pituitary-gonadal axis, the hormonal signaling chain that governs testosterone production. Where it affects testosterone at all, the effect appears to run through weight loss rather than through any direct hormonal mechanism.

How tirzepatide works: appetite signaling, slower gastric emptying, and insulin regulation
Tirzepatide's primary actions: appetite signaling, slower gastric emptying, and insulin regulation.

The Link Between Body Fat and Low Testosterone

Excess visceral fat, the fat that surrounds your organs rather than sitting under your skin, increases the activity of an enzyme called aromatase. Aromatase converts testosterone into estradiol. More visceral fat means more conversion, and less testosterone left in circulation.

Obesity also drives insulin resistance and low-grade inflammation, both of which blunt the brain's signal to the testes to produce testosterone in the first place. Clinicians call this functional or obesity-related low testosterone, and it's distinct from low testosterone caused by testicular or pituitary disease. For the full symptom picture, see our guide to recognizing the signs of low testosterone.

Pathway from sustained weight loss to less aromatase activity and better testosterone balance
The proposed pathway is indirect, and the size of the effect varies from person to person.

Reduce the fat mass, and some of this can reverse. That's the mechanism researchers are watching with tirzepatide and similar medications.

Does Tirzepatide Directly Suppress Testosterone?

No study has shown that tirzepatide directly suppresses testosterone production. Its dual action on GIP and GLP-1 receptors is still being studied for its full range of metabolic effects, and whether that dual mechanism changes the testosterone picture compared with GLP-1-only medications is not yet known.

What exists instead is a growing, if early, body of evidence pointing toward improved testosterone levels as weight comes off.

What the Research Shows in Men

The research on tirzepatide and testosterone is still forming. Most of it comes from conference presentations and small studies rather than large, peer-reviewed randomized trials. That matters, and it's worth saying plainly before looking at the numbers.

Real-World Data on Testosterone Levels With GLP-1 Medications

A retrospective records review presented at ENDO 2025 by researchers at SSM Health Saint Louis University looked at 110 men with obesity or type 2 diabetes who were not on testosterone or other hormonal therapy. The men were treated with semaglutide, dulaglutide, or tirzepatide. Over 18 months, with an average weight loss of about 10%, the share of men with normal total and free testosterone rose from 53% to 77%.

This was preliminary data presented at a medical conference, not a peer-reviewed published paper. It's a real-world sample rather than a controlled trial, so it can show an association but cannot prove the medications caused the change.

Tirzepatide vs. Testosterone Therapy in a Small 2025 Study

A separate, non-randomized study from the University of Catania, also presented at ENDO 2025, followed 83 men with obesity-related low testosterone. After two months, the tirzepatide group showed larger increases in LH, FSH, and both total and free testosterone than the groups using lifestyle changes alone or testosterone therapy. Testosterone therapy naturally lowers LH and FSH, so that part of the comparison is expected. Erectile function scores in the tirzepatide group improved more than in the lifestyle-only group and were similar to the testosterone therapy group.

Two months is a short window. Non-randomized means the groups weren't assigned by chance, which leaves room for other differences between them to shape the results. The findings are promising. They are not a green light to treat tirzepatide as a testosterone therapy.

Why the Evidence Is Still Early

A 2025 meta-analysis published in BMC Urology pooled four small studies using liraglutide and exenatide, both GLP-1 medications, and found that GLP-1 treatment was associated with higher bioavailable testosterone. Results for free testosterone and sex hormone-binding globulin (SHBG), a protein that binds testosterone and affects how much is active in your body, were inconclusive.

Tirzepatide is not FDA-approved to treat low testosterone, and none of this research changes that. If your testosterone is low, the path forward is testing and evaluation, not assuming a weight loss medication will resolve it on its own.

Does Tirzepatide Affect Libido or Sex Drive?

No clinical trial has established that tirzepatide lowers libido. What exists are anecdotal reports, not controlled data, and that distinction matters when you're trying to figure out what's happening to you.

Why Some People Report Lower Libido

A significant calorie deficit affects more than the number on the scale. Fatigue, nausea, and rapid weight change can all reduce interest in sex during active treatment, independent of any hormonal shift. And if your testosterone was already low before you started tirzepatide, that underlying issue doesn't disappear just because you're losing weight.

These are plausible contributors, not confirmed causes. Sorting out which one applies to you takes a conversation with a provider, not guesswork. Our sexual wellness page covers this territory in more depth.

When Libido Improves Instead

Some patients report the opposite. As weight drops and testosterone trends upward, libido and energy can follow. This tracks with the mechanism described earlier: less body fat, less aromatase activity, and potentially more circulating testosterone.

The honest takeaway is that the direction varies by individual, and the reasons why are not fully mapped yet.

Does Tirzepatide Cause Erectile Dysfunction?

This question deserves both sides of the evidence, presented plainly. Most of the data here comes from semaglutide and other GLP-1 medications rather than tirzepatide specifically.

What the Database Studies Found

A 2024 study using the TriNetX research database, published in the International Journal of Impotence Research, looked at non-diabetic men aged 18 to 50 with obesity who were prescribed semaglutide, a related GLP-1 medication. Compared with matched men who weren't prescribed it, the semaglutide group had higher rates of new erectile dysfunction diagnoses (1.47% versus 0.32%) and new testosterone deficiency diagnoses (1.53% versus 0.80%).

This was a retrospective database study. It can show a statistical association, but it cannot show that semaglutide caused the erectile dysfunction. Men who start a weight loss medication may also be more likely to see a doctor and get diagnosed with a condition they already had. That's called detection bias, and it's a real limitation of this kind of data.

The Case for Improvement

An ENDO 2025 meta-analysis pooling four studies and 396 men with type 2 diabetes or obesity found a trend toward better erectile function scores. That trend did not reach statistical significance, meaning it could reflect chance rather than a true effect. Add the Catania findings described earlier, and the picture is mixed rather than settled.

A 2026 review by Noakes and colleagues in the International Journal of Impotence Research concluded that weight loss modestly improves erectile function overall. It also noted that database signals like the TriNetX findings may reflect confounding or detection bias, while still deserving attention. Both statements can be true at once. That's what an honestly unresolved question looks like.

What to Do If ED Starts or Gets Worse

New or worsening erectile dysfunction during treatment is a reason to get evaluated, not a reason to assume the medication is responsible and stop on your own. A proper workup can distinguish between low testosterone, vascular causes, medication effects, and factors unrelated to tirzepatide entirely. Our erectile dysfunction page outlines what that evaluation involves.

What About Semaglutide and Other GLP-1 Medications?

Most of the testosterone research discussed above comes from GLP-1 medications broadly, not tirzepatide specifically. Semaglutide, dulaglutide, liraglutide, and exenatide all appear in these studies, and the Saint Louis University data combined semaglutide, dulaglutide, and tirzepatide into one analysis.

That's an important caveat. When you see a headline claiming "GLP-1 drugs raise testosterone," it's usually describing a class effect built on a mix of medications, not a tirzepatide-specific finding. If you're considering compounded semaglutide as part of your weight loss plan, the same early, evolving evidence picture applies.

Tirzepatide and Testosterone in Women

The picture for women runs in a different direction, for a different reason.

Women With PCOS

Elevated testosterone is one of the hallmark features of polycystic ovary syndrome (PCOS), so a drop in testosterone is generally a treatment goal, not a side effect to watch for. A 2024 meta-analysis of four placebo-controlled trials in women with PCOS and obesity found that GLP-1 medications, mainly liraglutide and semaglutide, produced a significant reduction in total testosterone. A 2025 meta-analysis of 13 trials, comparing GLP-1 medications mostly against metformin, found no significant difference in testosterone between the two.

Those two findings aren't necessarily contradictory. Comparing a medication with placebo answers a different question than comparing it with another active treatment. No completed tirzepatide trials in PCOS exist yet, so this evidence comes from other GLP-1 medications. Our guide to hormone therapy for women covers testosterone's role in female hormonal health more broadly.

Women Already on Hormone Therapy

If you're already on testosterone or another hormone therapy and you lose a significant amount of weight on tirzepatide, your lab values can shift. That's not a reason for alarm. It is a reason to recheck labs with your provider rather than assume your current dose still fits.

Your first visit is a quick blood draw. Your provider reviews the results with you at a second visit about a week later, so you know what to expect going in.

When Low Testosterone Does Not Improve With Weight Loss

Not every case of low testosterone is obesity-related, and weight loss won't fix the ones that aren't. Testicular causes, pituitary conditions, certain medications, and age-related decline can all produce low testosterone that persists no matter how much weight comes off.

What tirzepatide does and does not do, including why it is not a replacement for TRT

If your symptoms haven't improved after meaningful weight loss on tirzepatide, that's a signal to retest rather than to assume nothing can be done. A proper evaluation determines whether low testosterone treatment in Las Vegas makes sense for your specific labs and history. You can review the criteria that determine candidacy on our TRT candidacy page.

Getting Your Testosterone Tested in Las Vegas

Guessing at hormone status from symptoms alone leaves too much room for error. A complete panel shows what's actually happening.

What a Complete Testosterone Panel Includes

A proper evaluation starts with a morning blood draw, since testosterone follows a daily rhythm and runs highest early in the day. The panel should include:

  • Total testosterone: all the testosterone in your blood
  • Free testosterone: the unbound portion your body can use
  • SHBG: the protein that binds testosterone and affects how much is available
  • LH and FSH: the brain's signals to the testes, which help separate testicular causes from pituitary causes
  • Estradiol: a view of how much testosterone is being converted

An abnormal result deserves a repeat test before any diagnosis, since a single low reading can reflect poor sleep, illness, or timing rather than a true hormonal problem. Our guide to when to get your testosterone tested explains the process in more detail.

Frequently Asked Questions

Does tirzepatide increase testosterone?

It may, indirectly. Early studies in men with obesity show testosterone rising as weight comes off, likely because less body fat means less conversion of testosterone into estradiol. Raising testosterone is not an FDA-approved use of tirzepatide, and the evidence so far is small and short-term.

Can tirzepatide cause low libido?

No clinical trial has confirmed it as a direct effect. Reported drops in libido are mostly anecdotal and may relate to a large calorie deficit, fatigue, nausea, or testosterone that was already low before treatment started. Some people report better libido instead as weight and testosterone improve.

Does semaglutide lower testosterone?

In men with obesity, the available evidence mostly points the other way, with testosterone rising as weight decreases. One large database study did find more new diagnoses of testosterone deficiency and erectile dysfunction in younger non-diabetic men prescribed semaglutide, but that type of study cannot show the medication caused them.

Does tirzepatide lower testosterone in women?

It hasn't been studied directly yet. In women with PCOS, related GLP-1 medications have shown mixed results: one meta-analysis found a significant drop in testosterone compared with placebo, while another found no difference compared with metformin. In PCOS, lower testosterone is usually a wanted change.

Should I get my testosterone checked before starting tirzepatide?

If you have symptoms of low testosterone, such as fatigue, low libido, or trouble building muscle, a baseline panel is worth it. It gives you and your provider a starting point to compare against if your levels shift. If you're considering compounded tirzepatide in Las Vegas, ask your provider whether a hormone panel makes sense at the start.

Can I take tirzepatide if I'm already on TRT?

Many patients use both under provider supervision, with labs monitored along the way. How the two are coordinated is covered in our guide to combining TRT and tirzepatide.

Do brand-name versions like Mounjaro or Zepbound affect testosterone differently than compounded tirzepatide?

There is no data either way. Brand-name and compounded tirzepatide share the same active ingredient, but the studies discussed above used approved brand-name products. Compounded tirzepatide is not FDA-approved, is not the same product as the brand-name versions, and has not been studied separately for its effect on testosterone.
