Where Filler Goes, and What Each Placement Changes

These get confused constantly, and they are not variations on one treatment. Lip filler adds hyaluronic acid gel inside the lip and creates actual volume. A lip flip uses a small amount of neurotoxin in the orbicularis oris above the upper lip, relaxing the muscle so the border rolls slightly outward. Nothing is added. The lip only appears fuller because more of it is showing.
A flip lasts six to eight weeks against six to twelve months for filler, and it does nothing for a lip that genuinely lacks volume. What the Appointment Actually Involves
Consultation and assessment

Good candidates are adults with thin lips, age-related thinning, asymmetry, or volume loss, in general good health and not pregnant or breastfeeding. Men and women both are candidates, and the treatment goals are frequently different.
This is also where the volume conversation happens: whether a half syringe or a full syringe fits what you are asking for, and whether the answer is to stage it across two visits. An assessment that talks you out of a full syringe is doing its job.
Treatment day
Topical anesthetic, usually a compounded benzocaine, lidocaine, and tetracaine cream, sits on the lips for 20 to 30 minutes beforehand. All four of our fillers also carry premixed lidocaine, so the tissue continues numbing as the product goes in. The injection itself takes 15 to 20 minutes. Teresa uses both needle and cannula depending on the zone: body for volume, border for definition, commissures for support.
Immediate results and settling
You will see a change the moment you sit up, but you are not seeing your result yet. Swelling adds temporary fullness that peaks in the first 24 to 48 hours and then recedes. The settled outcome shows at about two weeks, once the filler has integrated and the swelling has fully resolved. Judging your lips on day two is the single most common source of unnecessary panic.
Half a Syringe or a Full Syringe
A syringe holds one millilitre. That is roughly a fifth of a teaspoon, which is less than most people picture, and it is why the overfilled look almost always comes from repeat treatments stacked too close together rather than from a single large one.
The conservative approach is not a sales tactic. Starting at half a syringe and layering at a follow-up costs more in appointments than committing to a full syringe at once. It also produces fewer results people ask us to dissolve, and it is the lower risk path for migration.
Lip Filler Aftercare
Downtime is minimal, but the first two weeks decide how well the result holds.
- First 24 hours No strenuous exercise, alcohol, saunas, or extended heat and sun. Cold compresses applied gently, not pressed. No kissing, straws, or puckering, all of which put pressure on product that has not begun to integrate.
- 24 to 48 hours Swelling peaks and then starts to fall. Bruising, if it appears, generally clears in 7 to 10 days, and clears more slowly if you were taking blood thinners, fish oil, or high dose vitamin E beforehand.
- First two weeks Do not massage or manipulate your lips. Skip retinol, glycolic acid, and aggressive exfoliants near the mouth, and be gentle brushing your teeth. Book nothing you would be self-conscious at.
- Ongoing Stay hydrated, limit UV exposure, and know that smoking measurably accelerates hyaluronic acid breakdown and will shorten how long your filler lasts.

Is one syringe or half a syringe right for me?
It depends on your baseline lip size, how much change you want, and your existing symmetry. Half a syringe, 0.5 mL, works well for border definition, subtle enhancement, or correcting minor asymmetry. A full syringe, 1.0 mL, suits a genuinely thin lip with little structure or a goal of noticeable volume.
Teresa frequently recommends starting with less and layering at a follow-up. Conservative first dosing lowers the risk of both overfilling and migration.
What causes lip filler migration, and how is it avoided?
Migration is filler moving beyond where it was placed, most visibly as a ridge or shelf above the lip border. The usual causes are overfilling, injecting too superficially, using a product with high water uptake in a lip that cannot accommodate it, and stacking treatments before the previous one has integrated.
Prevention is unglamorous: conservative volume per session, correct depth, appropriate product selection, and spacing sessions at least four weeks apart. Existing migration can be dissolved with hyaluronidase before retreating.
Does lip filler hurt?
Most people describe pressure with brief sharp moments rather than sustained pain. Topical anesthetic sits on the lips for 20 to 30 minutes first, and all four of our fillers contain premixed lidocaine that numbs the tissue as product is delivered. A dental block can be added for patients with low pain tolerance.
Is lip filler permanent? Can it be dissolved?
No, and yes. Hyaluronic acid fillers are broken down by your own enzymes over 6 to 12 months. If you dislike the result or filler has migrated, hyaluronidase dissolves it, usually within a day or two. We stock Hylenex for exactly this, and dissolving is a bookable appointment type rather than something you have to ask a favour for.
The FDA classifies hyaluronic acid fillers as temporary, absorbable devices. Permanent lip fillers exist but carry materially higher complication risk and are not used here.
How long does swelling last, and when do lips settle?
Swelling peaks in the first 24 to 48 hours and largely resolves within a few days. Bruising, when it happens, takes 7 to 10 days. At two weeks the filler has integrated and what you see is what you have.
If you have an event, treat at least two weeks ahead. If you are two days post treatment and unhappy, wait, because you are looking at swelling rather than at filler.
Can I get lip filler while pregnant or breastfeeding?
No. There are no clinical trials establishing the safety of dermal fillers during pregnancy or breastfeeding, so the standard of care is to postpone. If you are planning a pregnancy or currently nursing, raise it at consultation and we will talk about timing.

Wait at least 24 hours. Both apply pressure to product that has not begun integrating and can shift placement. Gentle contact is fine after a day; save anything vigorous for the end of the first week.
