Notice of Privacy Practices
Body Balance Medical is committed to protecting the privacy of your health information. This Notice applies to the Protected Health Information (“PHI”) we create and maintain as your healthcare provider and explains how that information may be used and disclosed, and your rights with respect to it.

We are required by law to maintain the privacy of your PHI, to provide you with this Notice of our legal duties and privacy practices, to notify you following a breach of unsecured PHI, and to abide by the terms of the Notice currently in effect. We reserve the right to change this Notice and to make the revised Notice effective for all PHI we maintain.
How We May Use and Disclose Your PHI Without Authorization ↑ Top
Treatment
We use and disclose your PHI to provide, coordinate, and manage your healthcare — for example, sharing information among our clinicians, with a compounding pharmacy that prepares your prescription, or with a laboratory that processes your bloodwork.
Payment
We use and disclose your PHI to obtain payment for the services we provide — for example, to process your payment method, administer financing you have applied for, or document services rendered.
Healthcare Operations
We use and disclose your PHI for operations such as quality assessment, clinician review, training, and business management.
Other Permitted or Required Uses
We may use or disclose your PHI without your authorization when permitted or required by law, including for: appointment reminders and treatment alternatives; as required by law; public health and safety activities; health oversight activities; judicial and administrative proceedings; law enforcement; to avert a serious threat to health or safety; workers’ compensation; coroners, medical examiners, and funeral directors; organ and tissue donation; certain research with appropriate safeguards; and specialized government functions.
Uses and Disclosures That Require Your Written Authorization ↑ Top
The following will be made only with your written authorization, which you may revoke at any time in writing:
- Most uses and disclosures of psychotherapy notes (if any).
- Uses and disclosures for marketing purposes, beyond communications permitted by law.
- Disclosures that constitute a sale of PHI.
- Use of photographs or testimonials that identify you, including before-and-after images.
- Other uses and disclosures not described in this Notice.
Your Rights Regarding Your PHI ↑ Top
- Right to inspect and copy — request access to and a copy of your PHI, including electronically where readily producible.
- Right to amend — request that we correct PHI you believe is inaccurate or incomplete.
- Right to an accounting of disclosures — request a list of certain disclosures we have made.
- Right to request restrictions — request limits on use or disclosure, including restricting disclosure to a health plan for services you paid for in full out of pocket.
- Right to confidential communications — request that we contact you at a specific location or by a specific method.
- Right to a paper copy — obtain a paper copy of this Notice at any time.
- Right to be notified of a breach of your unsecured PHI.
- Right to opt out of fundraising communications, if we conduct fundraising.
State-Law Protections ↑ Top
Where state law provides greater protection for your health information than HIPAA — including, for California residents, the Confidentiality of Medical Information Act (CMIA), and similar protections under Nevada law — we will follow the more protective standard.
Complaints ↑ Top
If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
Changes to This Notice ↑ Top
We may change this Notice, and the revised Notice will apply to all PHI we maintain. The current Notice will be posted on our Website and available at our clinic, with its effective date.
